The Village Health Outreach Program (Sanjeevani)

Piramal Swasthya, an initiative of Piramal Foundation works on bridging public healthcare gaps by supporting Government of India’s vision to meet Universal Health Coverage. With a focus on Maternal Health, Child and Adolescent Health, and Non-communicable Diseases, it has a decade-long experience in building and implementing healthcare innovations at scale to address the primary healthcare needs of the most underserved populations across 21 States of India.
Digital Bharat Collaborative (DBC), a part of Piramal Foundation, supports the government to transform the public healthcare system by building a robust digital delivery platform that ensures availability and accessibility of quality healthcare for all. Through this, we seek to enhance experiences of citizens in accessing government health services at the last mile.
In collaboration with the Government of Assam, we will be implementing The Village Health Outreach Program which aims in nearly identification, screening, referral, follow up and free medicine for effective management of lifestyle and non-communicable diseases (Diabetes, Hypertension, Defective Vision etc.), chronic diseases (Tuberculosis, Asthma, Epilepsy, Malaria etc.) along with Ante Natal Care Services and Minor ailments under National Health Programs in the 30 districts of Assam serving at least two settlements of 1500 population each day according to a pre-determined calendar for 23 days in a month.

| § To arrange or organize the counter for the Lab Technician as per the process

§ Coordination of the Service at the SP § To arrange or organize the Pharmacist's counter as per the process § Drug Dispense - OTC & Schedule H (on the basis of Prescription) § If medicine dispensed on Prescription, then Pharmacist to coordinate with R&MO to have the Prescription captured in the Web Cam and further in the laptop as specified in the roles of the R&MO § OP Card Filling (Relevant Section Only) § CD Attendance Register Maintenance (As & When applicable) § Critical Case Register Maintenance - As & When applicable § Beneficiary Drugs Issue Register Maintenance with relevant correct information § Back-Up of ANM in Basic Diagnosis, BP Measurement & Suggestion to L.T. for lab tests § Counseling & Referral of beneficiaries as and when applicable /required and in absence of ANM § Ensure the availability of Van Calendar and Referral Calendar in the VAN § Ensure the availability of Symptomatic CD Questionnaire/booklet at Service Point § Ensure the availability of Drug Dosages guide at SP § To acknowledge the BIN Card on receipt of Drug Caret from TMT every morning before leaving to Service Point § Submit all the information pertaining to Drugs Consumption and Remaining Stock at the Bin Carets to AM/AS by every EOD. § To maintain & submit Asset damage report (ADR) to AM/AS against any damages within 12 hours or same day of the actual damage/malfunction. § Ensure service begins as per schedule for morning & afternoon shifts § Ensure all Van staff have clean uniforms § Any other task assigned based on operational need. |

§ To ensure the availability of the following at the Service Points § Stake Holder's Attendance Register Maintenance at Service Point § Beneficiary Registration both System & manual § In case of Manual registration at service point happening out of any technical issue, R&MO to pass the entries in the R&MO application the same day after arriving at the Area Office § Responsible to coordinate beneficiary information from all counters in Application § OP Card/CD Card Filling (Relevant Section Only) § To arrange or organize his counter as per the process § To ensure that all related Assets are in good and working condition § To maintain & submit Asset Damage Report (ADR) to AM/AS against any damages of equipment pertaining to R&MO within 12 hours or same day of the actual damage/malfunction § To capture the photo & details of prescriptions in the Application as well as in a Folder specifying the concerned type of Document & date-month in the laptop, e.g., Prescription>>Month>>Date of Visit. § Reporting of the Stock of OP Card/CD Card to AM/AS by Month End which will be triggered as soon as the stock of Fresh OP Cards/CD Cards reaches a threshold value of 30% of the entire month's average consumption. | § To ensure MHU Siren is ON from the entry point of the Service Point
§ Beneficiary Mobilization. After dropping the Team at the set Service Point location, Pilot to move around the Village within a radius of .5 Km/500 meters with putting the Siren on to mobilize Beneficiary and will inform passersby to come and avail the Free Medical facility rendered by Sanjeevani VHOP § Beneficiary Queue Control - To ensure people are in Queue and only one beneficiary is allowed per counter § Van & its items' Safety § Keeping the Van Roadworthy & ability of understanding Road hazards § Log Book & Car Diary Maintenance § Helping R&MO in Height/Weight Measurement - Optional and only if Crowd is CONTROLLABLE § Pilots will utilize the Van CUG to call 104 SARATHI if any consultation with a Doctor from SARATHI or the Domain expert is required. § Fuel Filling to be Done only in the evening and in the presence of Pilot & AM/AS § Inverter Battery Charging to be ensured on a Daily basis § Regular checking of Distilled Water Level in the MHU and Inverter batteries to be ensured and relevant reporting to be done to AM/AS and Fleet Supervisor § Proper and appropriate assessment of the Van's running condition to be done for Month End Van Servicing § Any formats of reporting to be adhered which may be introduced by higher authority in future during the operations § Ensure that the service Van leaves the Area Office by 8.30 AM & reaches the service point by 9.00 AM keeping the safety & security of the van & co passengers intact. |

| § Innovation in Trend Analysis through customized MIS to prevent wastage happening in the Zonal Districts.
§ Relationship Management with Area Office Staffs, Zonal and SHQ Staffs § Collating the entire Districts' Meeting Schedule (Date & Time) planned between the AM/AS's and the District Health Authorities &/Or General Administrations and Present it to the Zonal Manager. § Accuracy in Database Management. § Random Calls to ASHA, Anganwadi, Gaonbura or CRP on the Service Delivery Feedback & Other validation by phone calls. § Adherence to Daily Reporting of Zonal Attendance to SHQ by 100%. § Empathetic Behavior towards peers § Timely Attendance Validation of the entire Zone. ZAA to coordinate the Final Attendance validation for all the Zonal Districts and follow up on cases marked as either Absent or LoP with individual districts and update status to HR accordingly. § Collation of Leave Requests from all the zonal districts, Emailing it to the ZM for Zonal level recommendation & sharing of the information sent back by the HRD with all concerned Districts. Also a Master Leave Tracker to be maintained. § Collation of the Daily Reports w.r.t. the VHOP and any ongoing additional project (MMU) from the Districts & maintaining the reports at the ZAA's end Month Wise. E.g., MHU, D&C, F12 (soft copy), Referral, Run Rate, WDF etc. § Follow Up with Area Managers & Supervisors to ENSURE timely submission of Weekly Bills to Zonal Office AND prompt escalation of any habitual delay of AM/AS to the ZM before the Finance Dept. raises a concern. § Attendance (System & Manual) and Punctuality. § Maintenance of the Monitoring & ANALYTICAL Reports (Most Crucial) and regular Sharing with the ZM. § Improving communication skills, interpersonal relations, work ethics and Public Relations Skills § Any other task assigned based on operational need. § Maintain coordination and good working relations with AM/AS and SHQ departments. § Ensure adherence to policies. |
